A CONTINUING PATH FORWARD

Support for the transition home.

Hospital discharge is an important transition, not a finish line. Some people benefit from ongoing psychotherapy as they rebuild routines, strengthen coping strategies, reconnect with relationships, and adjust to life outside the hospital.

Our work begins with understanding your current needs, existing supports, and goals. Together, we can focus on safety, emotional regulation, and practical skills before considering deeper therapeutic work when appropriate.

WHO WE SUPPORT

Different experiences. Individualized care.

Outpatient psychotherapy may be appropriate for children, adolescents, and adults who are medically and clinically ready for scheduled community-based therapy, including those living with overlapping mental health concerns.

01.

Children & adolescents

Emotional dysregulation, anxiety, trauma, neurodivergence (ASD, ADHD, AuDHD), school reintegration, family adjustment, and post-discharge transitions.

02.

Adults

Complex trauma, personality disorders, mood and anxiety concerns, emotional regulation, identity, relationships, and recovery following psychiatric hospitalization.

03.

Parents & caregivers

Understanding a loved one's needs, supporting emotional regulation at home, navigating changes after discharge, and strengthening communication and boundaries.

WHAT THERAPY CAN ADDRESS

Building stability, one step at a time.

Care is tailored to the person rather than a diagnosis alone. Areas of support may include:

Stabilization & regulation

Developing distress-tolerance skills, understanding emotional triggers, strengthening grounding strategies, and working with an existing safety plan.

Everyday functioning

Supporting the return to routines, school, work, relationships, and community activities at a manageable pace.

Trauma & complex experiences

Exploring the effects of difficult experiences with attention to readiness, consent, nervous-system capacity, and stabilization.

Family & support systems

Helping clients and their support networks understand patterns, communicate more effectively, and identify practical next steps.

OUR THERAPEUTIC APPROACH

Personalized care that meets you where you are.

Depending on individual needs and readiness, psychotherapy may integrate Dialectical Behaviour Therapy (DBT), Eye Movement Desensitization and Reprocessing (EMDR), Internal Family Systems (IFS), Emotionally Focused Therapy (EFT), and somatic or nervous-system-informed approaches.

Not every person needs trauma processing immediately after hospitalization. We prioritize collaborative assessment, stabilization, coping skills, and the pace that feels clinically appropriate.

Clinical fit matters. Services are offered by appointment and depend on availability and suitability. Théla is not a hospital outpatient department, intensive outpatient program, crisis service, or provider of medication management.

GETTING STARTED

Two ways to connect with Théla.

Clients and families can reach out directly. Healthcare and community professionals can also inquire about arranging care, including before an anticipated hospital discharge.

01.

Self-referral

Professional referral inquiry

Hospital social workers, discharge planners, case managers, physicians, psychiatrists, and community providers are welcome to contact Théla about possible outpatient psychotherapy and continuity-of-care needs.

Use our professional referral inquiry page to begin the referral process. Please share identifiable clinical records only through a confirmed secure submission channel.

Referral inquiries do not guarantee acceptance or appointment availability. Referring teams should maintain appropriate discharge planning, psychiatric follow-up, and safety arrangements until care is formally established.

COLLABORATIVE CARE

Connected care, beyond the therapy room.

Outpatient psychotherapy can complement existing medical, psychiatric, hospital, school, and community supports. With appropriate consent, Théla welcomes clinically relevant collaboration to help make transitions more coordinated.

  • Communication with psychiatrists, physicians, and discharge teams
  • Coordination of therapeutic goals with existing care plans
  • Collaboration with social workers, case managers, and community providers
  • Involvement of parents or caregivers when appropriate and authorized
  • Recognition of when more intensive or urgent services are needed

FOR REFERRING TEAMS

Preparing a referral inquiry?

Useful initial information includes the referring professional's name and organization, secure contact details, whether discharge is upcoming or completed, the broad reason for referral, and confirmation of authorization to share information.

Detailed clinical records can be discussed through an appropriately secure process once suitability and consent are established. Open the professional referral inquiry form →

COMMON QUESTIONS

Before you reach out.

Do I need a referral from a hospital or physician?

No. Individuals and families may contact Théla directly. Professional referrals are also welcome.

Can a caseworker inquire before discharge?

Yes. A professional can inquire about possible availability and suitability while discharge planning is underway. An inquiry does not replace hospital discharge planning or confirm that care has been accepted.

Can Théla work with my psychiatrist or case manager?

Where clinically appropriate, and with the necessary consent, relevant communication and coordination with other providers may be part of care.

Does Théla provide emergency or crisis services?

No. Théla provides scheduled psychotherapy, not emergency assessment, 24-hour monitoring, or crisis response. For immediate danger, call 911 or go to the nearest emergency department. In Canada, call or text 988 for suicide crisis support.

Is outpatient psychotherapy right for everyone after discharge?

Not necessarily. The level of support needed varies. A consultation helps explore fit, but some clients may need more intensive or specialized services.

YOUR NEXT STEP
